Upon application, PVP-Iodine slowly liberates free iodine (I2). The polyvinylpyrrolidone polymer component assists in delivering iodine to the target cell surface while mitigating its potential toxicity to mammalian cells.

For inflammatory skin conditions like acne, a 3% liposomal hydrogel showed potential with relatively better skin tolerance. For wound care and pre-surgical disinfection, 5% to 10% solutions are commonly utilized.

PVP-Iodine is classified as an over-the-counter (OTC) antiseptic. Common side effects include mild skin irritation, redness, dryness, and discomfort at the application site. Rarer reactions can involve pruritus, erythema, or small blisters, with very rare severe allergic reactions reported. Strong or overuse of solutions can be corrosive, potentially leading to irritant contact dermatitis. Systemic absorption is possible when applied to large wounds, which can lead to major systemic adverse effects such as metabolic acidosis, cardiovascular instability, renal failure, and thyroid dysfunction. It is not recommended for women less than 32 weeks pregnant, individuals with existing thyroid problems, kidney problems, or those taking lithium. It is also not recommended for newborn infants due to the risk of skin and thyroid issues. Safe concentrations for oral cavity use are up to 1.25%, and for nasal cavity use up to 2.5%.
