Piperonyl Butoxide exerts its action by inhibiting the mixed-function oxidase (MFO) system, also known as the cytochrome P-450 system, within insects. This enzyme system is crucial for insect detoxification of insecticides. By blocking these enzymes, PBO permits higher concentrations of insecticides to persist, thereby amplifying their lethal effects on insects. Crucially, research suggests PBO does not significantly impact the MFO system in humans, and no mechanism for cosmetic skin benefits has been identified.

Piperonyl Butoxide holds FDA approval as an adjuvant in specific human drug products for the treatment of head, pubic, and body lice. The US EPA classifies PBO as 'possibly carcinogenic to humans' (Group C) based on liver tumors observed in mice at high ingested doses; however, scientific analysis indicates these events are unlikely to occur in humans. The FAO/WHO Joint Meeting on Pesticide Residues concluded that PBO is not considered carcinogenic to humans at internationally accepted maximum tolerated doses. The WHO classifies its acute hazard in normal use as 'unlikely'. In the EU, its harmonized classification (ATP18, 2022) highlights it as very toxic to aquatic life with long-lasting effects, causes serious eye irritation, and may cause respiratory irritation.
