Mechanism of Action
The extract works by delivering a complex array of bioactive substances, including Epithelial Growth Factor (EGF), basic Fibroblast Growth Factor (bFGF), Platelet Derived Growth Factor (PDGF), Hepatocyte Growth Factor (HGF), Keratinocyte Growth Factor (KGF), and Growth Differentiation Factor-11 (GDF-11). These components stimulate the proliferation, differentiation, and regeneration of dermal fibroblasts, promote keratinocyte migration, and modulate immune responses. Its action leads to increased collagen and fibroblast presence, enhanced angiogenesis, reduced inflammation, and activation of the Wnt/β-catenin signaling pathway for cell proliferation. Furthermore, it inhibits melanin production by regulating microphthalmia-associated transcription factor (MITF) expression via the ERK signaling pathway, strengthens the skin barrier by reducing transepidermal water loss, and functions as an antioxidant, humectant, and skin protectant.

Clinical Evidence
Medium confidenceN/A
Key findings
- 01 Improved skin barrier function and reduced inflammatory responsiveness in patients with sensitive skin, without severe adverse events.
- 02 Promoted primary wound healing regeneration, increasing fibroblasts, collagen, angiogenesis, reducing inflammation, and stimulating hair follicle creation.
- 03 Accelerated proliferation of Human Dermal Fibroblasts (HDFs), increased Collagen type 1, 3 and Elastin gene expression, and decreased MMP-1 expression, contributing to skin rejuvenation and reduction of skin wrinkles in vivo.
- 04 Strengthened skin barrier and decreased transepidermal water loss (TEWL) when applied to lesions of patients with mild Atopic Dermatitis for 4 weeks.
- 05 Enhanced skin fibroblast proliferation and inhibited melanin production in vitro, demonstrating more effective skin regeneration than adipose-derived MSC-CM in aged skin in vivo.

Formulation
Stability
Typically supplied as a cryo-dehydrated powder, it requires reconstitution with a cosmetic base. The mixture has shown stability for approximately one week under refrigerated conditions.
Safety
Safety Profile
The Cosmetic Ingredient Review (CIR) has not evaluated this ingredient. In Europe, this ingredient is classified as 'Regulated, Forbidden in Europe' by CosIng. The US Food and Drug Administration (FDA) regulates regenerative medicine products, and unapproved products containing human umbilical cord blood components (including conditioned media and exosomes) require FDA licensure/approval for marketing to consumers. The FDA has reported adverse events, including blindness, tumor formation, and infections, due to the use of such unapproved products. Currently, only blood-forming stem cells derived from umbilical cord blood are FDA-approved for specific blood production disorders, not for general cosmetic use. While non-clinical safety evaluations in animal models have indicated topical safety, further human clinical studies are still needed to ensure its safe use for anti-aging purposes. Clinical studies on sensitive skin reported no severe adverse events.

Verdict
Despite promising preclinical and early clinical data for skin rejuvenation and barrier enhancement, significant regulatory prohibitions in Europe and explicit FDA warnings regarding unapproved stem cell-derived products and documented severe adverse events render this ingredient unsuitable for precision skincare formulations.
